ElderLynk, which was founded in 2000, is a model rural mental health outreach program that targets underserved adults over the age of 65. Following an evidence-based case management/interdisciplinary team approach for clinical services, ElderLynk works toward implementing a locally accessible and seamless mental health delivery system that is well coordinated and integrated with primary care services. This program provides community education and awareness programs about mental health. Located in Kirksville, MO, the program also trains local health professionals and helps to develop new models of mental health service delivery for older adults in rural areas.

Using case managers and coordinators from the local community, ElderLynk strives to overcome barriers to treatment and ensures culturally competent services. A faith-based outreach program is being implemented to enhance support within the community for those with mental illness. Consumers and their family members are encouraged to provide feedback and suggestions for mental health services. To improve the quality of care and to increase awareness among health care providers of mental health conditions, the program is expanding professional education programs to identify and encourage mental health practitioners who are committed to the region.
